Military Personnel Flight
We are the Military Personnel Flight at McConnell AFB. Our mission is to provide quality personnel support. We handle everything from ID cards to base in-processing, assignments, re-enlistments, and retirements for all Airmen and their families. We are also the one-stop source for evaluations, promotions, awards, and decorations, or other special actions.
As the MPF, we also handle the distribution of CACs and other ID cards. To apply or update your card, please make an appointment online or by phone via the Customer Support drop-down menu below.

REQUIRED FOR CAC REISSUANCE:
- All CAC reissuance customers are required to present 2 forms of valid ID (from the “List of Acceptable Documents” found HERE). Please bring these documents with you to the ID card Office.
- Your current CAC CANNOT be used as a valid ID. You must provide two other acceptable IDs, unless they already exist in your DEERS record.

To reset your CAC Pin or unlock a CAC, please visit the DEERS office between the hours of 8:00 AM – 2:30 PM.

What is a USID?
Issued to Reserve and retired service members, dependent family members, and other eligible individuals for benefits, privileges, and facility access.
For the Sponsor
- Must have CAC or DOD Self-Service Login access
- DEERS contact information must be up to date
For the Requester
- DEERS contact information must be up to date
- DEERS photo was taken in the last 12 years
- DEERS address is in the U.S. or APO/FPO/DPO

Required Documents/Items:
- Passport application
- DD Form 1056 for a new passport
- DD Form 82 for a renewal
- DO NOT SIGN. You will finalize the form with the passport agent during your appointment.
- Proof of U.S. citizenship
- Original U.S. Birth Certificate (with raised seal)
- Certificate of Naturalization or a previously issued U.S. passport
- Photocopy of the front and back of the CAC
- Orders or Approved Justification Memo (MILO)
- Passport photos
- 2×2 inches in size
- Taken within the last 6 months
- Plain white or off-white background
- Head must be 1-1 3/8 inches from the bottom of the chin to the top of the head
- Head must face the camera directly with full face in view
- No head coverings, eyewear, or uniforms (see exceptions below)*
- Print on matte or glossy photo quality paper
*Head coverings are not allowed unless accompanied by a signed statement confirming that they are part of traditional religious attire worn publicly or required daily for medical reasons. Eyewear is also not permitted unless a doctor provides a signed statement explaining why it cannot be removed (e.g., post–eye surgery). Photos must be taken in everyday clothing; uniforms, uniform-like clothing, and camouflage are not permitted.
